- What username and password should I use to login to complete my Family Relationship Disclosure form?
The Family Relationship Disclosure form requires the use of your District EAD username and password. If you do not remember it or need to have it reset, please contact the IT Help desk at (972) 925-5630.
- What should I do if I keep encountering an error message when trying to play videos?
Ensure you are using and logged into Google chrome as your browser for optimal viewing.
If you are receiving error messages, please refresh your computer.
At the beginning of the compliance period, a high volume of employees are watching videos and this may cause some video errors.

- I am having issues when completing the policy acknowledgment?
When completing the policy acknowledgment you must have pop ups enabled on your device. If using a mobile device, you must be able to close the pop ups. It is best to use a computer, laptop or tablet device because they have a larger screen.
- Why can't I watch the videos on my celluar phone?
The system is not compatible on a cellular phone or tablet. Please use a laptop or desktop computer.
